Thank you for your interest in serving as a Small Group Leader at Journey Church. Small Groups are at the heart of how we help people grow in their faith and build meaningful relationships, and leaders play a vital role in creating that environment.

This application is designed to help us get to know you better, understand your faith journey, and ensure we can best support you as you step into leadership. Please take your time to answer honestly and thoughtfully. Your responses will remain confidential and will be used to prayerfully guide our leadership team as we consider your application.

REQUIREMENTS TO LEAD A JOURNEY GROUP

Pastoral Reference *

As part of the application process, we ask each Small Group Leader candidate to provide the name of a pastoral reference.

Growth Track *

We require all small group leaders to complete Growth Track before the launch of their group.

All Journey Group leaders must have a current, clear background check on file with Journey Church to lead a group.

REQUIRED INFORMATION FOR GROUP SET UP

The information you provide will be used to create your group on the Planning Center. Once created, your group will be visible as a Journey Group that others can view and request to join.

Please provide the name of your group. (Example: John and Jane Smith's Couples Group)

Where will your group meet? *

Please provide the location(s) where your group will meet. (Example: at a group member's home, at a local coffee shop, etc.).

How often will your group meet? *

Please provide the frequency that you plan to meet. (Example: 2nd and 4th Fridays of each month/unless a holiday).

What time will your group meet? *

Please provide the time that your meeting will start and end. (Example: 6:30pm-8:00pm)

Provide a short description of your group. *

Please provide 1 or 2 sentences that give some details about your group. Include what activities and studies you are doing or are planning to do. (Example of description might be: "Fellowship, Bible Study, sporting events (social) using RightNow Media for our study of growing in faith").

What will your group study? *

What will you use as your curriculum? S.O.A.P., the Bible, RightNow Media, discuss the most recent Sunday morning teaching from your campus?
